Proof of identification for yourself and/or your spouse
Social security cards for you, your spouse and dependent. Or Individual Tax Identification number (ITIN)
Birth dates for you, your spouse and dependents
Wage and earning statements: forms w-2, w-2g, 1099-r, from all employers
Interest and dividend statements from bands (form 1099)
Bank routing number and account number for direct deposit. This can be found on your checks
Other relevant information about income and other expenses
Total amount paid for day care and the day care providers tax identification number (the individuals social security number of the business' employer identification number)
Documentation to substantiate any deductible items
Copy of last year Federal and State return
Remember, your spouse must be present if filing jointly!
